Submission Checklist

The following is to be used as a guideline for the minimum amount of information necessary to secure a quote. Individual Carriers may require additional information on a case-by-case basis.

General Liability

  • Acord Application (125 & 126-including prior carrier information)

  • 5 years of hard copy loss runs valued within the last 90 days

  • Contractors Supplemental Questionnaire (company specific may be required at binding)

  • OCIPs (Wraps) – Wrap Application, Soils Report and 5 years loss history of Project GC.

  • Products Supplemental Application (if applicable)

  • Additional information (resumes for new ventures, explanation of operations, etc.)

Excess / Umbrella

  • Acord Applications (125, 126 and 131 – including underlying carrier, premium, term and policy limit for CGL, Auto and EL).

  • Description of operations for each named insured.

  • 5 years of GL hard copy loss runs and 3 years of Auto, valued within 90 days of submission.

  • Contractors Supplemental Questionnaire (if applicable)

  • Products Supplemental Questionnaire (if applicable)

  • Any additional information such as layering for a higher excess carrier placement.

Professional E&O / Healthcare / Environmental

  • Acord Application (125)

  • E&O / Environmental application (Specific Supplemental may be required. Please contact us for assistance.)

  • 5 years of hard copy loss runs valued within the last 90 days.

Property / DIC

  • Acord Application (125 and 131) including:
    age of building, square footage, construction, number of stories, protection class, building improvements and date updated and any recent inspections or pictures

  • 3 years of hard copy loss runs valued within 90 days of submission.
